The Hospice Team

Skirball Hospice care is provided by an interdisciplinary team of professionals and volunteers. Our team works with a patient's physician and family to create a highly individualized plan of care to control pain, manage symptoms and meet palliative care goals.

The Skirball Hospice team includes the following:

  • Medical Director
The Medical Director (physician) directs care and consults with the hospice patient's family and personal physician. The medical director supervises the work of other team members to ensure the plan of care is being met, and is available to make home visits when warranted by a change in clinical condition.

  • Nurse
A Registered Nurse coordinates patient services. Direct nursing services include assessment, administering medications for symptom management, evaluation of disease processes, supportive care, and providing other treatments as ordered by the physician.

  • Social Worker
A Social Worker provides patient and family with counseling and emotional support, and assistance with advance directives, living wills, Do Not Resuscitate orders, coordination of community resources, and funeral pre-arrangements.

  • Spiritual Counselor
A Rabbi or Chaplain offers spiritual guidance and support for the patient and family.

  • Registered Dietitian
A Registered Dietitian assesses the nutritional needs of the patient and provides education and information.

  • Certified Home Health Aid
A Certified Home Health Aid provides the patient with personal care, including assistance with showering or bathing, skin care, grooming, and personal hygiene needs.

  • Volunteers
Trained volunteers provide comfort and companionship, run short errands for the patient, or do grocery shopping for the family.

  • Therapists
If warranted by the patient's clinical condition, physical, occupational, and speech therapists are available to enhance quality of life.

  • Bereavement Counselor
A trained professional and specialist provides grief support for hospice families and coordinates grief support groups for the general community and hospice caregivers.
